Detailed Description

Worker-death lost set: issued = [0, bound), lost = issued minus the union of the collected batch histories — a batch in no history was issued but never completed (its claimant died, or the map fn errored mid-batch); a dead runner's whole history lands here too — it publishes only at exhaustion.

spans (caller-assembled from the histories, element space, lo <= hi) is sorted in place; the gaps (at most n + 1) land in out as 0-based half-open ranges; returns the gap count.
